Job Description: Physical Therapist (PT)

Position Overview:

We are seeking a highly skilled and compassionate Physical Therapist (PT) to join our healthcare team. Key Responsibilities:

• Patient Examination and Evaluation: Examine patients with impairments, functional limitations, disabilities, or other health-related conditions to determine a diagnosis, prognosis, and appropriate intervention.
• Initial Exams and Data Evaluation: Perform and document initial exams, evaluate data to clarify and substantiate patient complaints, problems, and functional limitations.
• Individualized Treatment Plans: Plan, prepare, and carry out individually designed physical treatment programs to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction.
• Progress Documentation: Record prognosis, treatment, response, and progress in patient charts. Identify and document goals, anticipated progress, and plans for reevaluation.
• Treatment Evaluation and Adjustment: Evaluate the effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Testing and Measuring: Test and measure patient’s strength, motor development and function, sensory perception, functional capacity, or respiratory or circulatory efficiency and record data.
• Patient Instruction and Education: Instruct and educate patients on a plan of care to be continued independently.
• Therapeutic Interventions: Design, implement, and modify therapeutic interventions to alleviate impairment and functional limitations.

Qualifications:

• Education: Copy of diploma/transcripts supporting graduation with a Doctor of Physical Therapy (DPT) degree from an accredited program.
• Licensing: Copy of a current, valid, unrestricted license as a PT issued by the Arizona State Board of Physical Therapy.
• Verification: Copy of current, completed PSV from the Arizona State Board of Physical Therapy obtained via Arizona State Board of Physical Therapy.
• Fit Testing: Copy of current, completed fit testing for N95 particulate respirators.
• Experience: Minimum of one year of past work experience as a Physical Therapist.
